The tripeptide γ-L-glutamyl-L-cysteinylglycine, better known asglutathione (GSH), is often referred to as the body’s master antioxidant.

Primarily synthesized in the liver by the sequential addition of cysteineto glutamate followed by glycine, it can also be synthesized in nearly alltissues of the body,1,2 and is present in millimolar concentrations in allmammalian cells.3 Glutathione functions as a key intracellular antioxidantin its reduced, active form, where it neutralizes free radicals and issubsequently oxidized to glutathione disulfide (GSSG), then recycled backto GSH, maintaining cellular redox homeostasis.*3

In addition to playing a key role as a direct antioxidant in managingoxidative stress, GSH also has numerous other cellular functions, includingas a cofactor for enzymes in detoxification pathways, in the maintenanceof immune system function, for cell-cycle regulation, and the recycling ofother antioxidants and nutrients.*2,3

The maintenance of GSH tissue levels is critical for optimal health,2 and anincreased risk of numerous chronic conditions is linked to GSH depletion.

Many lifestyle and exposure factors can influence GSH levels in bloodand tissues, which vary widely across tissue types.2 To date, moderatedepletion of the intracellular GSH pool has been correlated with impairedimmune function, exposure to oxidants/drugs/toxins, poor nutrition thatlimits availability of precursor amino acids, increases in oxidative stressthat occur with aging, and excessive exercise.*2,4,5

Antioxidant and Detoxification

As the master  antioxidant, the primary role of GSH is to neutralizereactive oxygen species (ROS), an umbrella term for a group of highly reactive molecules that can be produced intra- and extra-cellularly. As the drivers of oxidative stress, ROS can cause damage to proteins, lipids, nucleic acids, and biological membranes. Glutathione helps to maintain reduction-oxidation (redox) balance, the steady state between oxidation and antioxidant protection that is essential for protecting from oxidative damage that destabilizes tissues and accelerates cellular aging.1 In addition to being a direct antioxidant, GSH also helps regenerate other key antioxidants, particularly vitamins C and E. This recycling process enables stronger, more efficient systemic antioxidant activity.*3

Glutathione also acts as a detoxification agent by conjugating with toxins, drugs, and heavy metals through enzyme-mediated reactions. The sulfhydryl group (−SH) of the cysteine moiety of GSH is the functional component involved in conjugation and reduction reactions that facilitate the removal of peroxides and many xenobiotic compounds,1 converting undesirable molecules into water-soluble compounds that can then be excreted from the body through bile or urine.*6

Cellular Functions

As an agent for redox balance, GSH plays a vital role in maintaining mitochondrial function. By helping stabilize lipids within mitochondrial membranes, it supports membrane integrity, slows mitochondrial aging, and enables more efficient energy production while minimizing oxidative damage.*1,7

Beyond its effects on mitochondrial health, GSH is involved in a wide range of essential cellular processes, including protein folding, protection of protein thiols from oxidation and crosslinking, enzyme regulation, degradation of proteins containing disulfide bonds, and regulation of the cell cycle and proliferation, underscoring its importance as a multifunctional regulator of cellular health.*3

Immune Function

Exerting effects on both the innate and adaptive immune systems, GSH plays a central role in supporting immune function. It has been shown to enhance the activity of key immune cells, including natural killer (NK) and T cells, which are critical for identifying and eliminating pathogens and abnormal cells.8,9 Lymphocytes also require GSH to function effectively, as even modest fluctuations in intracellular GSH can significantly influence their behavior.*4

Some research suggests that GSH supplementation may enhance NK cell activity and overall immune function; however, there is limited indication that supplementation further enhances immune function in healthy populations.2,4,8,9 What is evident is that depletion of intracellular GSH has been shown to impact immune cell function,4 underscoring its essential role in immune health.*

Glutathione Bioavailability

Because GSH is a critical antioxidant that readily shifts between its reduced (active) and oxidized forms, effective supplementation may depend on delivery method. Early research using oral GSH demonstrated minimal improvements in cellular levels or oxidative stress markers, reinforcing concerns about poor bioavailability. It was found that GSH is vulnerable to enzymatic degradation in the gastrointestinal tract. This process can break it down into precursor components and significantly limit the amount of active GSH that is absorbed.10,11 Emerging evidence suggests that use of liposomal technology, where GSH is encapsulated within lipid-based vesicles (liposomes) that act as protective carriers, may resolve this limitation.

Liposomes help shield GSH from harsh digestive conditions and enzymatic breakdown, allowing a greater proportion of the active compound to be absorbed and potentially leading to improved clinical outcomes. Several randomized, controlled trials have suggested that oral liposomal glutathione supplementation increases systemic GSH levels, enhances antioxidant status, and may support immune function.*2,8

In a randomized, double-blind, placebo-controlled trial in healthy adult subjects (N = 54), daily doses of 250 and 1,000 mg of liposomal GSH given

over a 6-month period resulted in dose- and time-dependent increases in GSH across whole blood, erythrocytes, plasma, lymphocytes, and buccal cells, with levels returning to baseline after a 1-month washout period, indicating a direct effect from supplementation. This study also reported improved oxidative stress markers, including a lower oxidized-to-reduced GSH ratio. Additionally, after 3 months of use at the higher dose, an increase in NK cell cytotoxicity was observed, and although not statistically significant, trending changes in lymphocyte activity were noted.*2

Similar results from a pilot study complement the prior study’s findings.

Healthy adult subjects (N = 12) were given oral liposomal GSH at 500 or 1,000 mg/day over a 1-month period. Dose-dependent increases in GSH levels were noted within 1 week, with peak GSH elevations at 2 weeks of up to 40% in whole blood and 100% in peripheral blood mononuclear cells.

These increases were accompanied by significant reductions in oxidative stress biomarkers, including a 20% decrease in the oxidized-to-reduced GSH ratio. Additionally, immune function improved markedly, with NK cell cytotoxicity rising by up to 400% and lymphocyte proliferation increasing by up to 60% within 2 weeks.*8

In further evidence of GSH’s ability to ameliorate oxidative stress, a 3-arm trial was conducted to evaluate the effects of oral GSH in both its reduced (GSH) and oxidized (GSSG) forms on skin biomarkers in healthy, middle-aged, female subjects (N = 60) with a daily GSH dose of 250 mg over 12 weeks.

Participants received GSH, GSSG, or placebo, with assessments performed at baseline and throughout the study period. Notably, GSH supplementation led to significant reductions in wrinkle appearance at select anatomical sites, while both forms of glutathione were associated with improvements in skin elasticity across sun-exposed and protected areas. These findings contribute to evidence suggesting a role for glutathione in modulating visible signs of skin aging, likely a result of its antioxidant activity.12 Glutathione supplementation may represent a practical, systemic approach that provides essential antioxidant protection from the oxidative damage that accelerates cellular aging.*1

Collectively, these studies contribute to the body of research suggesting that oral liposomal GSH supplementation elevates both intra- and extra-cellular GSH levels, reduces susceptibility to damage from oxidative stress, and may favorably modulate immune cell function. Liposomal GSH supplementation represents a promising strategy for improving systemic GSH status, supporting overall health, and potentially mitigating the risk for conditions associated with glutathione depletion.*2,8
